Seasons Medical’s Clinics Earn National Recognition For Patient-Centered Care
Rexburg, ID – The National Committee for Quality Assurance (NCQA) announced recently that Seasons Medical’s Adult and Family Care, Family Medicine, and Pediatrics Clinics have received NCQA Patient-Centered Medical Home (PCMH) Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and long-term, participative relationships.
“Our mission’s natural alignment with the NCQA standards to coordinate and deliver patient-centered care is a great honor,” said Seasons Medical’s Board Chairman Robert Meredith, DO. “We are proud of our clinics in receiving this important recognition in healthcare from the NCQA.”
Practices that become recognized under Patient-Centered Medical Home Recognition have demonstrated commitment to patient-centered care and clinical quality through: streamlined referral processes and care coordination with referring clinicians, timely patient and caregiver-focused care management and continuous clinical quality improvement.
“NCQA Patient-Centered Medical Home Recognition distinguishes practices that communicate, collaborate and integrate care in ways that patients want and that improve quality,” said NCQA President Margaret E. O’Kane. “I commend the team at Seasons Medical for its achievement, and for its commitment to continuous improvement.”
To earn recognition, which is valid for three years, Seasons Medical’s Adult and Family Care, Family Medicine, and Pediatrics Clinics demonstrated the ability to meet the program’s key elements, embodying characteristics of the specialty practice. NCQA standards aligned with the joint principles of the Patient-Centered Medical Home established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association.
