Vaccination tracking Update

All students, trainees, volunteers, and/or other individuals who provide care, treatment, or other services for the Hospital and/or its patients under contract or by other arrangements are required to report their vaccination status for both Flu (highly recommended) and Covid-19 (recommended). Please respond to the questionnaire table found later in this application process.   If you have further questions, please contact your school or organization for further information.
